Category: Injury and Claims
Date First Published: 29 July 2008
Summary: This form is new and is completed where a worker secures a job under the WorkSafe Incentive Scheme for Employers (WISE).
Related information
WISE is a 12 month financial incentive of up to $26,000 that is payable to employers who offer ongoing employment of 15 hours or more to workers with a WorkSafe Victoria (WorkSafe) entitlement who cannot return to work (RTW) with their pre-injury employer.
Employers who are not eligible for WISE are labour hire companies, employment agencies or group training organisations that place workers into a workplace of another employer.
Workers cannot access WISE for themselves. This includes self-employment or if you are a director or board member of a company that makes you an offer of employment.
Employers are offered WorkSafe Injury Insurance Protection if the worker stops work because of a new WorkSafe claim. As well, the cost of the claim is excluded from the employer’s injury insurance premium calculation. The employer is liable, however for the first 10 days of weekly payments and the first $564 (correct as at July 2008 – indexed annually) of medical expenses.
To give employers the added confidence that they have chosen the right applicant, an occupational rehabilitation provider will complete a workplace assessment to confirm the worker’s capacity and skills match the job and the workplace.
More information regarding WISE including what payments are made to the WISE employer are found in the publications below.

How to complete the form
The Placement Form contains all the necessary information to enable the WorkSafe Agent responsible for managing the worker's claim to record that the worker has secured a new job with a new employer under WISE. The recording of a WISE registration by the Agent enables the WISE financial subsidy to be paid to the WISE employer. In most instances, an OR provider will complete the form and provide a copy to the Agent and the WISE employer. Where an OR provider has not been involved, the Agent should forward a form to the WISE employer as soon as they are advised that the worker has secured a job under WISE.
Where to send the form
A copy of the Placement Form is forwarded to the WorkSafe Agent responsible for managing the worker's claim and the WISE employer.
